New Patients
Steps to becoming a new patient:
- Please contact our office through the contact form below or by phone message. We will reach out to you within 1 business day to to schedule your first appointment and answer any questions you might have.
- For all patients, we require a credit card on file in order to hold your new patient appointment. If you need to cancel or reschedule this appointment, please do so more than 24 business hours before your scheduled visit time.
- If your new patient appointment is cancelled less than 24 business hours before the scheduled time or you do not show, we will apply a $50 cancellation fee to your credit card on file. We pride ourselves on offering extended appointment times for new patient visits, and therefore as a business model cannot we afford to have large open time slots that could have been offered to other patients if the appointment had been canceled on time. This policy may be waved on a case-by-case basis for true emergencies.
- Once you establish as a patient you will be invited to our Online Patient Portal, which gives you access to our online forms and questionnaires, visit summaries, handouts, documents and more.
- At least one business day prior to being seen by the doctor, you will need to complete all of our online forms and upload any recent medical records so that the doctor has sufficient time to review these before your visit.
- For insurance patients, please note that we do not verify insurance benefits. We will send you insurance verification forms before your visit that you must review with you insurance company to verify that the doctor is in-network.
